Our solution is currently piloted in two large hospital systems. We have strong experience in nursing deployment, and the solution is designed to work with, instead of to replace, existing quality / care transition workflows already deployed in hospitals.

Ringful Health solution consists of 3 major components: 1. a tablet (iPad or Android) based device for in-patient patient education, discharge planning, and clinician communication; 2. a multi-modal and rule-based patient engagement solution to follow up patients for symptoms, signs, and self-care questions via ubiquitous devices the patients already own at home; 3. a predictive analysis and machine-learning system to stratify risk for individual patients and alert case managers via integrated dashboards to manually follow up high risk patients.

The questionnaire and rules are evidence-based, and they are completely customizable by healthcare providers to adjust to specific patient populations. The machine learning aspect of the system ensures that risk stratification is always based on best available data, including never-before collected data such as self-care behavior markers and patient reported outcomes. The solution is secured with industry strength cryptography and is HIPAA compliant.
